Job Description

What You’ll Do

  • Provide and coordinate patient care in accordance with physician orders
  • Oversee, guide, and support LPNs and caregivers in daily clinical activities
  • Develop personalized care plans in collaboration with patients, families, and healthcare providers
  • Educate and assist families in understanding diagnoses, treatments, and care plans
  • Maintain accurate, timely documentation and ensure clear communication across the care team
  • Participate in an on-call rotation to support patients and staff as needed

What We’re Looking For

  • Graduate of an accredited school of professional nursing
  • Current RN license and registration in New York State
  • Valid driver’s license
  • Strong commitment to compassionate, patient-centered care
