Primary Care & IV Infusion

Job Type: Full-Time, Part-Time, or PRN
Setting: Primary Care / Outpatient Clinic / Infusion Services
Schedule: Day shifts; weekday and PRN schedules may be available

Position Summary

We are seeking an experienced Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN) with a strong background in primary care and IV infusion therapy. The nurse will provide direct patient care, assist providers with clinical procedures, administer medications and IV therapies within the applicable scope of practice, monitor patients, and support the daily operations of the clinic.

The ideal candidate is dependable, patient-focused, organized, and comfortable caring for patients with acute and chronic medical conditions in a fast-paced outpatient environment.

Key Responsibilities

• Provide nursing care to patients in a primary care and outpatient setting.

• Perform patient intake, vital signs, medication reconciliation, and medical history review.

• Prepare patients for examinations, treatments, and procedures.

• Start and maintain peripheral IV access when permitted by state law, employer policy, training, and competency.

• Administer prescribed IV fluids, medications, vitamins, antibiotics, and other infusion therapies within LPN/LVN scope of practice.

• Monitor patients before, during, and after IV infusions.

• Recognize and promptly report adverse reactions, infiltration, extravasation, allergic reactions, or changes in patient condition.

• Discontinue peripheral IVs and provide appropriate post-infusion care.

• Administer oral, intramuscular, subcutaneous, and other authorized medications.

• Perform injections, immunizations, blood glucose testing, specimen collection, and basic point-of-care testing.

• Assist with wound care and dressing changes.

• Support management of patients with chronic conditions such as hypertension, diabetes, asthma, and cardiovascular disease.

• Assist physicians, NPs, PAs, and other clinical staff during examinations and procedures.

• Review provider orders and ensure treatments are completed accurately.

• Provide patient education regarding medications, treatments, follow-up care, and provider instructions.

• Respond to patient calls and communicate clinical concerns to the appropriate provider.

• Maintain accurate and timely documentation in the electronic medical record (EMR/EHR).

• Follow infection-control, medication-safety, HIPAA, and clinic policies.

• Maintain emergency supplies and infusion equipment and ensure appropriate infection-prevention practices.

Minimum Qualifications

• Current and unrestricted LPN or LVN license in the state of practice.

• Graduate of an accredited practical or vocational nursing program.

• Minimum 1 year of nursing experience preferred.

• Previous primary care, family medicine, internal medicine, urgent care, outpatient, or ambulatory care experience preferred.

• Previous IV infusion and peripheral IV experience required or strongly preferred.

• Demonstrated competency in IV insertion and infusion monitoring where permitted within the applicable LPN/LVN scope of practice.

• Current BLS/CPR certification.

• IV therapy certification or documented IV competency preferred and required where applicable.

• Ability to recognize and respond appropriately to changes in patient condition.

• Strong communication, documentation, and organizational skills.

• Comfortable using electronic medical record systems.

Physical Requirements

The position may require prolonged standing and walking, bending, reaching, assisting with patient transfers, and lifting or moving supplies and equipment. Reasonable accommodations may be provided as required by applicable law.

Important Scope-of-Practice Requirement

All IV therapy, medication administration, assessments, and other nursing duties must be performed in accordance with the applicable state Nurse Practice Act, LPN/LVN scope of practice, provider orders, organizational policies, and demonstrated clinical competency.
