Job Description

Job Summary

Caregivax is seeking a compassionate and dependable Licensed Practical Nurse/Licensed Vocational Nurse to provide direct nursing care in accordance with the patient's plan of care, provider orders, facility policies and applicable scope-of-practice requirements.

Responsibilities

Obtain and document vital signs and clinical observations.
Contribute to patient/resident assessments.
Administer medications and treatments as authorized.
Perform wound care and dressing changes within scope and competency.
Monitor patients for changes in condition.
Report significant clinical changes promptly to the supervising RN or provider.
Implement assigned portions of nursing care plans.
Collect specimens when authorized.
Assist with admissions, transfers and discharges.
Provide patient and family education within scope.
Maintain accurate EHR/medical-record documentation.
Follow medication-safety and infection-prevention procedures.
Participate in emergency response.
Maintain patient privacy and confidentiality.
Collaborate with RNs, physicians, NPs, CNAs and other members of the healthcare team.

Qualifications

Current unrestricted LPN/LVN license for the state of practice or applicable multistate privilege.
Graduate of an approved practical/vocational nursing program.
Current BLS/CPR certification.
Knowledge of medication administration and nursing fundamentals.
Strong communication and documentation skills.
Ability to recognize and promptly report changes in patient condition.
EHR experience preferred.
Previous healthcare experience preferred but may vary by employer.
