Job Description

Group Home Supervisor include overseeing staff within an assigned group home, developing and implementing Individualized Service Plans (ISPs) and behavior support plans, and presenting these plans to staff, families, and guardians. The supervisor ensures the group home complies with state licensing regulations and is also responsible for tracking and documenting each client’s progress using the organization's electronic recordkeeping system.

Position may, at times, include evenings and weekends.
$650 Sign on Bonus ($250 paid out at 2 months & $450 paid out 4 months)
VALLEYLIFE is currently searching for a Full-time Residential Group Home
DDD (Department of Developmental Disabilities) experience REQUIRED.

Minimum Job Requirements:

  • AZ Driver's License
  • 2 years experience working with developmental disabilities.
  • 2 years experience as a supervisor in group home or days services.
  • Be able to lift 40 -50 pounds.
  • Be able successfully pass a background check that meets State Licensure requirements.
  • Be able to document provided services accurately and efficiently.
  • Possess basic computer skills.
Schedule:
  • 8 hour shift
  • On call
  • Weekends as needed

Experience:
  • Group Home : 2 years (Required)
  • Supervisor in a group home: 1 year (Required)
  • DDD : 2 years (Required)

License/Certification:
  • Driver's License (Required)
  • Ability to Commute
  • Phoenix, AZ 85021 (Required)
