Job Description

  • Performs ongoing evaluations of patients and families, promptly recognizing and reporting deviations from expected outcomes.
  • Monitors patient responses to treatments, education, and discharge plans, ensuring accurate documentation and timely communication with the healthcare team and families.
  • Adheres to medication administration guidelines, documentation standards, and regulatory requirements to promote optimal patient safety.
  • Educates and orients patients and families on hospital procedures, systems, and expectations to support informed participation in care.
  • Delegates and supervises care activities based on staff competency, providing constructive feedback and evaluation to maintain high safety and quality standards.
  • Collaborates with physicians, nurses, and families to develop, implement, and update individualized care plans; actively participates in patient rounds and care conferences.
  • Fosters a culture of learning by exemplifying patient-centered care and mentoring students and new staff through education and clinical support.
  • Performs additional responsibilities as needed to ensure effective patient care delivery and efficient unit operations.
