Current CPR certification required.
Completion of at least 12 hours of annual in-service education.
Experience in geriatric nursing, emergency care, and first aid preferred.
Training in caring for cognitively impaired residents (e.g., Alzheimer’s and dementia care) highly desirable.
Ability to manage the mental and emotional demands of the role.
Strong interpersonal skills with the ability to collaborate effectively with physicians, leadership, staff, and external partners.
Physical capability to bend, stoop, climb, reach overhead, kneel, balance, push, pull, squat, crouch, lift, carry, and move up to 100 pounds.
Duties include, but are not limited to:
Obtain and document vital signs; promptly notify supervisors of any deviations according to the Individual Service Plan (ISP).
Assist residents with activities of daily living (ADLs), including:
Personal hygiene (bathing, grooming, hair care, shaving, and skin care)
Bed-making and linen changes
Toileting and incontinence care
Monitoring nutritional and fluid intake
Dressing
Ambulation and transfers
Proper positioning
Medication reminders
Additional duties within scope of practice as assigned
Follow each resident’s Individual Service Plan (ISP) and daily task sheets.
Communicate changes in resident condition or home environment to the appropriate supervisor.
Prepare and submit accurate and timely progress notes.
Maintain a safe, clean, and supportive living environment for residents.
Demonstrate compassion, respect, and engagement in all resident interactions.
Maintain confidentiality in compliance with HIPAA regulations.
Attend bi-weekly in-service trainings and all mandatory meetings.
Comply with all community and company policies and procedures.
